A caregiver's honest account of what finally worked — and the psychology behind why streaks change behaviour when guilt and alarms can't.

Second, there is identity formation. Research by BJ Fogg and others in the habit formation field shows that people who track visible progress begin to incorporate the behaviour into their self-concept. My dad no longer thinks of himself as someone who has to be reminded to take his medications. He thinks of himself as someone who has a 60-day streak. That identity shift is more durable than any external reminder.
Third, streaks create what psychologists call a "hot streak" bias — when you are on a roll, you want to stay on a roll. Consecutive success in any domain creates momentum that makes the next instance of that behaviour more likely, not less. Duolingo reports that streak loss is the single strongest predictor of user churn. Users who lose a streak are significantly more likely to abandon the app entirely than users who maintain one. I want to be careful here, because there is a version of this conversation that is condescending — the idea that elderly people need to be gamified into taking their medications, as though they were children. That is not what I am describing.
What I am describing is this: elderly patients managing multiple chronic conditions often feel that their relationship with medication has become defined by failure. A missed dose, a bad reading, a hospitalisation, a doctor's disappointed expression. The accumulated weight of this framing — medication as burden, non-adherence as moral failure — makes the entire category psychologically aversive.
Streaks reframe this completely. They make medication-taking a cumulative achievement rather than a daily obligation. They give an 80-year-old with four medications and a busy week something to be proud of rather than something to feel guilty about. In my experience, this reframing matters enormously for the population that struggles most with chronic medication adherence.

Celebration-based systems work from the opposite assumption: the problem is insufficient recognition of what the person is doing right. You took your medications 23 days in a row. That is genuinely impressive for a person managing multiple chronic conditions with a complicated life. Acknowledge it. Make it visible. Make the next day feel like an opportunity to extend something valuable, not an obligation to avoid punishment.
Streak systems can create anxiety around breaking a run — especially for patients who are already prone to health-related anxiety. A well-designed streak system resets gently, without punitive language, and treats the first dose after a miss as the start of something new rather than a reminder of failure.
